- Identify the members of your Caring Community. It may include many, or just two - you and a friend, relative or neighbor.
- Get addresses and phone numbers and emergency contacts for each person.
- Get to know one another; meet at a house of worship, library, senior community center, or someone’s home.

- Define your goals and expectations. Be realistic. It’s better to begin with a small, achievable goal and build on it, than to have big ambitions that never become reality.

- Identify your strengths and capabilities
- Identify the needs in your community or target population
- Find out what kinds of resources are already available in your neighborhood.

There are many forms a Connected Caring Community can take. It may be as simple as a phone tree, or a buddy system; or it could be a neighborhood collaborative to tackle projects like yard work, weatherization or snow shoveling.
